表皮生长因子在翼状胬肉切除术后的临床应用

表皮生长因子翼状胬肉切除术后的临床应用

目的  评价翼状胬肉切除+角膜缘干细胞移植术后应用表皮生长因子眼液对角膜创面修复、角膜混浊形成、角膜缘血管翳形成、角膜散光变化和预防胬肉复发的疗效。
方法 将翼状胬肉切除+角膜缘干细胞移植术后患者73例(90只眼)按随机方式分成3组,表皮生长因子组(金因舒眼液+氧氟沙星眼液)30只眼,阳性对照组(贝复舒眼液+氧氟沙星眼液)30只眼,阴性对照组(生理盐水眼液+氧氟沙星眼液)30只眼。术后随访时间1年,定期采用角膜荧光素染色观察角膜创面修复情况,眼前节图像采集系统观察角膜混浊形成情况、角膜缘血管翳形成情况和胬肉复发情况,手术前后分别用自动电脑验光仪检测角膜散光变化度数。
结果  术后角膜创面修复的时间表皮生长因子组和阳性对照组都比阴性对照组短(P <0.01),角膜荧光素染色面积表皮生长因子组和阳性对照组都比阴性对照组小(P <0.01),但表皮生长因子组术后角膜创面修复的时间更短,角膜荧光素染色面积更小;术后角膜混浊的吸光度表皮生长因子组比阳性对照组和阴性对照组都大(均P<0.01),而阳性对照组与阴性对照组组间差别无统计学意义(P>0.05);术后角膜混浊的面积、角膜缘血管翳面积、角膜散光变化的度数和胬肉复发率表皮生长因子组比阳性对照组和阴性对照组都小(均P<0.01),而阳性对照组与阴性对照组组间差别无统计学意义(均P>0.05)。
结论  表皮生长因子具有促进角膜创面修复、减轻术后角膜混浊形成、角膜缘血管翳形成、角膜散光变化和预防胬肉复发的作用。表皮生长因子治疗翼状胬肉切除术后的临床疗效优于重组碱性成纤维细胞生长因子。
 
 
Clinical therapeutic effect of epidermal growth factor on postoperative pterygium

Objective  To evaluate the clinical therapeutic effect of the epidermal growth factor (EGF) eye drop on corneal wound restoration, corneal opacity, pannus corneae, change of cornea astigmatism degree and the recurrence of pterygium post pterygium excision with limbal epithelial autograft.
Method  73 patients (90 eyes) who have underwent pterygium excision with limbal epithelial autograft were divided into 3 groups randomly:30 eyes in the EGF group ( EGF eye drop +Ocuflox),30 eyes in the positive control group [recombinant basic fibroblast growth factor (rb-FGF) eye drop + Ocuflox],and 30 eyes in the negative control group (normal saline eye drop +Ocuflox ).The post operation follow-up period was one year.Corneal wound restoration was detected by fluorescein staining. Corneal opacity, pannus corneae and the recurrence of pterygium were detected by anterior segment image capturing system. Preoperative and postoperative cornea astigmatism degree was detected by automatic refractor.
Results The corneal wound restoration periods of the EGF group and the positive control group were shorter than that of the negative group (P<0.01). The area of fluorescein staining of the EGF group and the positive control group were smaller than that of the negative group (P<0.01).The corneal wound restoration period of the EGF group was shorter than that of the positive group and the area of fluorescein staining of the EGF group was smaller than that of the positive group. The absorbance of corneal opacity of the EGF group was smaller than that of the positive control group and the negative group (both P<0.01), while the difference between the positive control group and the negative group has no statistically significant (P>0.05). The area of corneal opacity, the area of pannus corneae, the change of cornea astigmatism degree and the recurrence rate of pterygium of the EGF group were smaller than that of the positive control group and the negative group (all P<0.01), while the difference between the positive control group and the negative group has no statistically significant (all P>0.05).
Conclusion The EGF eye drop can accelerate corneal wound restoration, lessen the degree of corneal opacity and pannus corneae, decrease change of cornea astigmatism degree and prevent the recurrence of pterygium. Clinical therapeutic effect of EGF on postoperative pterygium is better than that of rb-FGF.
